Template Questionnaires

An integral part of the Biomedical Multimedia Unit's Formative Evaluation Program is the use of template questionnaires. Educational technology developers are able to use these resources both as a guide for development and as a data collection tool.

As the name suggests, these questionnaires provide a template of potential questionnaire items. With the assistance of an evaluator, developers are able to select questionnaire items that suit the focus of their evaluation. Developers often include additional items based on the particular features of their educational technology program.

The template questionnaires are provided as PDF files below.

You will notice that the questionnaires are designed for three types of respondents: content experts, graphic design experts and users. In addition, questionnaires exist in both quantitative and qualitative formats, giving evaluators flexibility in the types of the data they wish to collect.

Aspects of these questionnaires can be used to form combined qualitative and quantitative formative evaluation questionnaires.

An example of this is a questionnaire constructed to evaluate the educational technology program Communicating with the Tired Patient. This program uses extensive audio and video and, in an initial version, was accompanied by a workbook.
